Membership enrollment is $25. Minimum membership period is 6 months, then month to month. We must have your credit card on file to charge on the 1st of each month. Ability to roll over benefits for up to 6 months, as long as you are a current member. All benefits expire once membership is cancelled. No service substitutions for monthly treatment benefit. Cancellations must be in writing at least 30 days in advance.
